This service can be extremely valuable when adult children live out of town or work full-time and can't assist their elderly parent as much as they would like to. A case manager can conduct an overall assessment of the client's needs, secure needed community services (emergency response system, home delivered meals, etc.), arrange doctor appointments, home repairs and modifications, lawn care and snow removal. A case manager can also provide weekly visits to nursing home or home bound elderly. They can do shopping for clothes, shoes, and personal supplies. Case managers can provide transportation and escort clients to doctors visits, tests, out-patient surgery at the hospital and assist with packing and moving to a retirement center. The case manager can provide updates to adult children or trust officers with the client's consent.
