 Inside Visiting Fact Sheet

  • The visiting must be in the resident’s room.
  • Appointments are no longer necessary.
  • A maximum of 3 visitors may visit the resident at a time.
  • Children are allowed; however, they must wear a mask and maintain 6 feet social distance from the resident.
  • Young children are to always remain with an accompanying adult while in the building.
  • Guests are to always wear a face mask and residents are to wear a face mask during the visit.
  • All visitors will be prescreened with questions from the CDC upon arrival and temperature is to be taken.
  • The screening is to occur in the front lobby. If no one is in the front lobby when you arrive, please call the Salem desk at 309-683-2515 and remain in the lobby until someone comes to assist you.
  • Visitors will be directed to the appropriate room for the visit and are not allowed in other parts of the building without authorization from administration.
  • If these guidelines are not followed the guest may be asked to leave the facility for the safety of the residents and staff.

 

 Outdoor Visiting Fact Sheet

  •  The visiting will occur primarily on the patio near the front of the building.
  • There will be a table located between the visitor and the resident to assist in maintaining 6 feet social distancing.
  • Appointments are no longer necessary; however, we do ask that you call the Front Desk or Nurses Station 1 hour in advance so the resident can be brought outside for the visit in a timely fashion.
  • A maximum of 5 visitors may visit the resident at a time however 6 feet social distancing is required.
  • Children are allowed; however, they must maintain wear a mask and keep 6 feet social distance from the resident.
  • All guests and residents are to wear a face mask during the visit.

Resident Leave Fact Sheet

  • Most residents are to quarantine in their room if they leave the building for a non-medically necessary trip for over 24 hours or if the trip warrants quarantine per IDPH guidelines. Each event will be reviewed on a case-by-case basis.
  • Residents are to sign out at the time of the leave and sign back in upon return.
  • The family is to contact the nurse’s station to notify the nurse when the resident is going out.
  • Residents are to wash their hands once they return to the building.
