We provide nursing care and rehabilitation therapy following a qualifying hospital stay for orthopedic surgery, stroke, neurological, pulmonary, and other medically complex conditions. We strive to provide the highest quality of therapy and nursing care to facilitate the transition back to home or to a more independent living setting.

The number of residents living in each neighborhood is small, which adds to the excellent individualized care we give. Our goal is to maintain consistent staffing: the same caregivers for each neighborhood. This allows us to really understand each person’s needs and give the highest level of personalized care. It allows you, as a family, to know the caregivers and have confidence in their abilities.
